Duty and Responsibility

• Identify patient service requirements with potential and actual patients and family members in a position to understand service requirements by applying nursing theory, clinical and evidence-based practices.

• Maintain writing updates on nursing guidelines, policies and procedures on time according to the nursing standard operating procedure (SOP)

• Assure quality of care by developing and enforcing standards and nurse practices that adhere to the professional nursing act requirements and governing body

• To formulate, implement and conduct a thorough nursing assessment of the patient’s physical, psychosocial, cultural, and spiritual needs.

• From time to time, communicate with the management team, supervisor and family members on client progress.

• Demonstrate “hand-on” clinical skills based on the nursing standard operating procedure (SOP) in the area of practice home care.

• Ensure all patients are treated as individuals, irrespective of colour or creed.

• Maintain sensitivity to all individual’s religious and cultural practices.

• Able to evaluate client responses to interventions to determine progress towards achieving expected outcomes and modify the nursing care plan together with a nurse supervisor.

• Provide comprehensive report when transferring/handing over the accountability of a client’s care to another nurse.

• To be held accountable for the professional actions of the team during the span of duty.

• To establish a caring and therapeutic relationship with the client by demonstrating trust, respect and honesty.

• Create a positive environment by listening and offering support, ensuring maintaining confidentiality at all times.

• Assist family in identifying psychosocial needs, concerns and provide appropriate resources through information provision, education and nursing care knowledge.
